We will be asking every patient if they are happy to share their medical record with other users of the same clinical system. This is for the purpose of giving you the correct treatment if you attend a walk in centre or other healthcare provider.  

Success Centres at Moredon Medical Centre & Swindon NHS Health Centre - during periods of limited appointments patients may be offered alternative appointments at one of Swindon’s Success Centre's based at Moredon Medical Centre and Swindon NHS Health Centre. Reception staff will be able to book you directly into a booked GP appointment at the centre. These appointments are for minor ailments and not ongoing medical conditions. There are now additional GP appointments, operating on Saturdays and Sundays from 8am-8pm at Moredon Medical Centre and Swindon NHS Health Centre.

Text Reminder of Appointments - patients will now receive a text message reminder of their appointments at the surgery, the surgery would be grateful if patients could keep us informed of any changes to their mobile numbers.

Card Payments - Patients are advised that the surgery does not accept card payments. All payments should be by cash or cheque.

Named GP

As of 1st April 2015 all patients are to be allocated a named GP. For our patients this will be the doctor you normally see. Please enquire at reception if you wish to confirm these details are correct.

Electronic Prescriptions

The surgery is due to change its prescription service. In January 2015 Electronic Prescriptions will be implemented and patients are advised to contact their nominated pharmacy to register for the service. Further information regarding Electronic Prescriptions can be obtained from the surgery.

 NHS England - List Maintenance Project 

Registration letters from NHS England – please respond if you get one.

The NHS is carrying out work in Bath, Gloucestershire, Swindon and Wiltshire to make sure the patient details on GP lists are accurate. Letters are being sent to some patients, but not all. If you do receive a letter, please either confirm your details or submit any changes. If the details are correct, you can confirm by freepost, by Freephone, via text message or online. Changes can also be made by returning the form in the prepaid envelope.

It is important to respond to the letter, as GP lists do become out of date over time. If we don’t have an accurate record of your name and address, then we or a hospital may not be able to contact you with important information.

We may also need to contact you:

  • with test results
  • to invite you for bowel, breast or cervical screening to protect against cancer, for example
  • to offer vaccination against infectious diseases such as flu or measles, mumps and rubella
  • to give you an appointment as part of your plan to stay healthy if you have a long-term illness.

Please respond to the letter within four weeks, so the NHS can be certain about your details. If you do not respond, the NHS will assume that you have moved away, and begin the process to remove you from the practice list.
